Question: What is the best treatment for chronic cellulitis in the legs? has been getting 1 gram. vancomiacin in saline iv. twice daily plus rocephin once daily? Vancomiacin was discontinued, serrata is part of the picture now. The individual is in Ca. state prison.

I am so sorry to hear about this infection. With the causative germ identified, the laboratory normally tests to say which antibiotics would be best to treat. Am pretty sure this is what is guiding the choice of antibiotic. They initially started with many antibiotics and now that they have more precise information on the type and what antibiotic is best, they have discontinued Vancomycin which really has no effect on serratia. Rocephin is a good drug against this germ, serratia. To me, the treatment is adapted and I will recommend that you continue with it. Also, if you have the bacterial culture results, upload them for further insights and guidance.

Hello again: When I look at various websites concerning Cellulitis, MRSA, I see no specific association with Serratia. When I look at Serratia I see specific association primarily with "contaminated magnesium sulfate solutions ". Can you explain what a source of that type of infection might be? If he ( the incarcerated patient ) sends me his medical records relating to this issue I will forward them to you for our records if appropriate. Thank you for your help

Association with contaminated magnesium sulfate is prominent on the internet because that originated form a pharmaceutical company distributing contaminated magnesium sulfate by Serratia which resulted in several infections among clients consuming their products.
