Question: I have severe constipation, I am 89 yrs old. I have to dig out stool by hand. Severe cramping but no movement. Doctor gave me movantik, but no help. Take laxatives and softeners , still no help. XXXXXXX ( YYYY@YYYY )

I appreciate your concern

Based upon your history , you have tried most medication that have shown little response. In this case further evaluation is required. Local examination to look for anal fissure, hemorrhoids, fecal impaction etc should be done. X ray erect abdomen, serum electrolytes, thyroid profile, CBC, ESR , Renal function tests can be advised. Increased intake of fiber e.g ispaghula husk and fluids should be undertaken and use of milk and milk products, coffee , red meat, oily / fried food, tea should be limited to minimum. Regular exercise at least 30 minutes daily should also be made a routine, if possible. Prokinetics like tegaserod and stimulant laxatives e.g senna can be advised in cases of severe constipation along with stool softeners and movantik, if no other cause is isolated.
