Suggest Treatment For Chronic Cough

Question: I have chronic cough since last 2 years. Recently I consulted with allergiest and checked allergy with Total IgE 1200.2 with eosinophil 11. He is proposing to go for immunotherapy. What to do?

I can understand your concern.
Possibility of either asthma or bronchitis or lung infection is high in your case.
In my opinion, you should first consult pulmonologist and get done
1. Clinical examination of respiratory system
2. Chest x ray
3. PFT, (Pulmonary Function Test).
Chest x ray is needed to rule out lung infection.
PFT is must for the diagnosis of bronchitis or asthma. PFT will also tell you about severity of the disease.
You may need inhaled bronchodilators and inhaled corticosteroid (ICS).
Oral combination of antihistamine and anti allergic drugs are also useful in your case.
Immunotherapy is the last option for uncontrolled allergies. You should first try other things for allergies.

I consulted with pulmologist as well. Both pft and chest X ray done. Pft is normal. In chest xray nothing found except prominent vessels. I am a smoker recently gave up smoking. Doctor gave me three medicines flutiflo nasal spray, airtec puff and glemont f tablet.

I can understand your concern.
You might be having allergic rhinitis only.
Best treatment of allergy is identification of allergen and if possible complete avoidance of it.
Your IgE is raised. But IgE is general marker for allergy. Subsets of IgE are also tested for specific allergies like food, dust, smoke, chemicals, pollens, animal fur, dander etc.
So better to get done specific IgE. Also observe yourself from which you are allergic.
Immunotherapy is beneficial only if you avoid allergen. If allergens are continuously present then you have to repeat immunotherapy every year. And immunotherapy is not cheap. So better to avoid specific allergens as much as you can.
You are smoker and having allergies with coughing, it's strange to have normal PFT.
Can you please send me your PFT report in attachment, so that I can guide you better.

Thanks for your follow up question on HCM.
I have gone through the reports you have attached.
PFT report is normal.
Somewhere in the notes it is written that DNS is present.
Sometimes severe DNS (deviated nasal septum) can also cause such symptoms.
Amongst the susceptible antigens, aspergillus fumigatus is worrisome. It infect lungs and stays there. It is commonly misdiagnosed as asthma. On simple chest x ray it looks as prominent vascular markings (same as your case).
So for confirmation you need to get done HRCT (high resolution CT thorax).
So please let me know
1. Have you ever undergone HRCT thorax?
2. Have you ever consulted ENT doctor for DNS?

I already diagnosed with DNS and evaluated by ENT specialist. Within 3 months I will go for septoplasty. It is moderate DNS. I never done HRCT Throx. Is this aspergillus fumigatus dangerous. I searched in Internet. Am I immuno compromised. I don't have fever or other weakness. I had a risky sexual exposure long back. After 21 months I did HIV antibody test and result negative.

You should definitely get done septoplasty as early as possible, because in some cases this is the main contributing factor for your symptoms.
For aspergillus fumigatus, it is actually not a fungus. It is common in allergic patients. You are not immunocompromised. But this infection is common in XXXXXXX especially in allergic individuals.
It can cause ABPA (allergic bronchopulmonary aspergilosis). It is not life threatening condition but it is chronic condition.
In this HRCT is better than chest x ray. So to rule out this, you should definitely get done HRCT thorax.

Thanks for your answer. Just the final question:
Is high IgE affect HIV antibody test result? I did this test after 1 Year 9 months of possible exposure.

Is allergy a sign of poor immune system or just over reaction of Immune system?

No, raised IgE level has nothing to do with HIV testing.
If your HIV test after 21 months is negative then no need to worry for HIV infection. You are safe.
Allergy is not a sign of poor immunity.
It is actually seen in susceptible individuals who are more sensitive to some substance. So allergy is actually a protective phenomenon, to prevent those substances from getting into the body.
So allergy is not at all sign for poor immunity.
