Question : For about six months now, I've been suffering from chronic fatigue and the other symptoms I have is a very sore throat, my ears hurt and most usually my chest hurts as well. I have no energy to do anything and have to take pain medication to make it through my day. I've been through x-rays and have seen my family doctor on several occasions, but he has no idea what I have. The days that my condition is at it's worst, I have terrible nausea and have to go to bed because I am so sick. I have made an appointment with a specialist, but my appointment is over a month away.

I have also often had headaches, and the pain and discomfort that I go through is awful, and nobody seems to understand what I'm going through physically and emotionally.

If it helps at all, I did a mammogram exam last week, and they have already called my doctor because they found something and want me to go for an ultrasound, so I've already made an appointment for that.

Brief Answer:
Lifestyle modification needed

Detailed Answer:
Thanks for asking on HealthcareMagic.

I have gone very carefully through the details provided. You could be suffering from an upper respiratory tract infection. You need to gurgle with lukewarm water in order to relieve your sore throat. Taking steam inhalation can provide relief from your chest discomfort as water in form of vapour will reach deep inside and will help in dilute the thickened mucus. Take rest and plenty of fluids to drink. You may take naproxen 500 mg tablet when required, in case you have headaches and pain.

You also need to get your thyroid profile checked as low thyroid levels could be contributory.

Your chest pain could also be due to gastroesophageal reflux.

In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Take Prilosec (omeprazole) once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fiber intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* If I had a patient like you I would perhaps have suggested Prodep (fluoxetine) 20 mg tablet (long term anxiolytic) once daily for at least 2 months (it may take time for the action to set in).
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.
* Stress reduction has a very important role to play. Yoga and meditation should help.

Hopefully that should help.

Let me know if I could help further.

Hello Dr. Prof. XXXXX

Thank you so much for getting back to me so quickly, and with such a detailed answer! As you can imagine, being sick for so long, all I want is answers and a solution to relieve my pain and misery. You mentioned quite a few medications in your summary, would I need a doctor's prescription for them? Also, would you say that my chronic fatigue has to do with my condition? I did not mention in my list of symptoms, but I also have chronic bloating.

When I said that I have throat pain, it's not just a simple sore throat that I was referring to, my symptoms all together, including the throat, feels like I have tonsillitis; would that change my diagnosis at all? I sometimes wake up out of a deep sleep with the pain, and very often wake up to all the pain in the morning.

I look forward to hearing back from you at your earliest convenience.

Best regards,
XXXXXXX

Brief Answer:
Most of them are available over the counter

Detailed Answer:
Prodep is a prescription medicine. All the others are over the counter. None of the problems (that I discussed) are associated with chronic fatigue syndrome.

I would additionally insist you to visit a psychologist. He would be able to administer cognitive behavioral therapy which will help you cope better with your pain.

Even if you have tonsillitis, gurgling with lukewarm water would be the mainstay of treatment.

Feel free to write back.

Good morning Dr. Prof. XXXXXXX Saha,

What is Prodep exactly? What really bothers me is not only the pain, but the fatigue. Why is this condition making me so tired? I am not working at the moment, and I am scared to get another job because I'm scared of not being able to make it through a day with this fatigue, it's ruining me, so I'm wondering if this can somehow be cured?

Thank you so very much for helping me through this very trying time in my life.

Best regards,
XXXXX

Brief Answer:
Questions answered in sequence

Detailed Answer:
Thanks again for writing back. I have already mentioned that it is a long term anxiolytic although it has a number of other uses. You need to get your thyroid profile checked as low thyroid levels could make you tired. A doctor could evaluate you clinically to understand whether it could be due to chronic fatigue syndrome. Based on the underlying cause he could offer suitable remedy. I have already mentioned that you can visit a psychologist and cognitive behavioral therapy could help you cope better with the pain.

Feel free to write back.
