Suggest Treatment For Chronic Migraine Headache

Question: Sir,
I have been having the problem of migraine headache for the past 8 to 10 years. ( I am 57 yrs now with no BP and Sugar problem, have been taking Eltroxin 25 mcg for thyroid problem and Rosutec 10 mg for cholesterol problem, both are normal now ) Initially I tried chrocin, saridon and like that and subsequently I have been taking Vasograine since last 3-4 years now. I take 1 tablet of Vasograine a day and it gives me relief for that day. I get migraine headache normally 4-5 days in a week. During DEC 2014 it was diagnosed as migraine and was prescribed Sibellium 10 mg for 6 months initially and then was advised for another 3 months. Initially it gave me a very good results and the frequency of attack reduced drastically. But of late it is not of much help as I am getting the attack again daily inspite of taking Sibellim 10 mg every might for the last 2 months now. I have to take again Vasograine in the morning to get relief for that day. It has become routine to take Vasograine in the morning and Sibellium in the night for this problem. My questions as below:

1) Since Vasograine gives me relief for that day, If I take regularly everyday 1 tablet at the onset ( as has been the case now ), does it affect Kidneys in the long run. If not, can I continue with this.

2) As a preventive medicine, Sibellium seems to be of not much help, is there any alternative medicine to get rid of the problem.

3) If I take Vasograine 1 tab let at the onset of the attack every day in the morning and as usual 10 mg Sibellim in the night, will it be alright or does this have any serious side effects in the long run.

Kindly advise.

Hello,
I have gone through your question and understand your concerns.
Migraine is a lifestyle associated disease.
The first step is to change lifestyle.
Take timely breakfast and lunch. Avoid excessive tea or coffee and chocolates.
Take adequate sleep.
Avoid any other triggers if identified.
Prophylaxis drugs are available like propranolol, Topiramate, Flunarizine, divalproate.
In case you are not having benefits with Sibelium then you can switch to amitryptiline with divalproate.
Regular intake of vasograin is not recommended for prophylaxis.
Pain killers should be taken less frequently as overuse may cause medication overuse headache.
Regular follow up with a neurologist is advised.
I will prefer 10 mg of amitryptiline and 500 mg of divalproate in my patients.
Yoga and relaxation exercises helps too.
Overuse of pain killers is harmful for kidney too.

You need prescription for medicines.
I prefer Tryptomer 10 mg initially to be increased after 10 days and divalprol 500 mg once daily.
The drugs are increased in dosage if required.

Sir,
Thanks for your quick advise. Kindly let me be advised as to when these medicines to be taken and is generic name Divalproex sodium same as that of brand name Divalprol. Should both, amitryptiline and Divalprol be taken together ?
