Brief Answer:
may need consider an oral antibiotic along with anti viral

Detailed Answer:
HI, thanks for using healthcare magic

As you would know cold sores do not normally last this long.

As your doctor stated it is possible that there may be a super infection present. Metrogel can treat certain types of bacteria but there are others that may not respond and may require oral antibiotics

Persistent swelling can sometimes indicate cellulitis which is essentially bacterial infection of the skin and underlying tissues.
You may need to consider an oral antibiotic along with the anti viral that you are using.

An anti inflammatory pain medication may also help to reduce surrounding inflammation and swelling even if there is no pain eg cataflam, naproxen

If the lesion still persists then your doctor may want to consider a complete blood count and also screening for any conditions that may affect the immune system eg diabetes, autoimmune diseases,

I hope this helps, feel free to ask any other questions
