Suggest Treatment For Congestive Heart Failure

Posted on Thu, 13 Apr 2017
Question: I have congestive heart failure. My right ventricle is leaking. About eight years I had my first Echocardiogram. I was told that my heart was working about 15 to 20%.I was rollerblading and playing tennis. I was able to get my heart up to 50-55%.This last August it dropped to 29%.I had the die injected. They found that my right side I had blockage. I was put on more med's. This has made very weak, and short of breath. I have lost my energy. Is there a way to repair the heart ? Like repairing the valve, possible unblocking the arteries.

Brief Answer:
Yes repair is possible . Risk vs . Benefit in your case requires discussion

Detailed Answer:
Hello Sir,
I am Dr. Mody and I would be addressing your concern .
I would request you to send me some more information so I can guide you objectively.
Do send me your latest EKG, 2D-echo cardiology report and what all medications are you taking.

Now coming to your query-

- In most probability due to the fall in pumping of the heart, there has been gradual dilatation of the heart.

When heart dilates the valves become incompetent due to the stretching of the annulus which supports the valve.

This causes back leak and the symptoms you described.

One form of treatment is medically offload your heart and hence decrease the back leak . The trade off here is the medications used has a diuretic as an important component which flushes the extra fluid from the body , this can cause a lot of weakness and the decrease pumping of the heart supplies less blood to the exercising muscle causing weakness . optimum titration of the medicine is the best way forward .

The next step is to find the reason for decreased pumping and reverse the same, this would automatically decrease the leak. Also, the arteries which are blocked can be taken care of.

Yes, surgical repair and intervention are also possible, however, each case has to be weighted on its merits and the best form of surgery is decided based on the risk vs. benefits considering your age of 72 years and the condition of the heart.

After reviewing all your reports will send you a more precise surgical option which would best suit your case.
