Suggest Treatment For Constant Cough And Fever In A Child

Question: from last night my daughter 3.3 years having continue cough not able to speak even..3_4 days back she started cough n fever. my local pedal given antibiotics in starting augmenting DDS then after two days taxim O has started..doctor given her chericoff..reliant..honeytuss..but no use from continuous cough..please suggest some medicine for instant relief from continue cough...we r doing nebulising of asthalin,dulion also 3_4 times a day.

Hi.... I understand your concern.

By what you say I feel that this is an acute exacerbation of wheeze and not a bacterial infection. I do not see the need for starting antibiotic and changing antibiotic is not at all required (especially Taxim-O as it will not work for respiratory infections at all - it is drug of choice for typhoid).

I have a few questions for you - please get back with answers so that I can guide you better.

Questions:
1. How many days per month does she cough or feel breathless?
2. How many nights per month does her sleep get disturbed due to above symptoms?
3. Does she feel breathless when she runs around or plays with other kids?
4. Are the symptoms when there are seasonal changes?
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
6. Is the cough always associated with fever?

1. usually once or twice in month for the period of 5-6 days.
2. 3_4 nights per month.
3. she does not feel breathless when play it run..
4. when season change..she get caught in cough or cold.
5. no history of asthma...
6. cough is always not associated with fever..only when viral infection..this time she caught with viral n started fever from last two -three days. it's usually occur when she is sleeping...yesterday night she was having 100 fever so I gave meftal p.

Looks like multi triggered wheeze - needs inhaled corticosteroids

Detailed Answer:
Hi...By what you say I feel that your daughter might be having a hyper reactive airways disease. It is resulting in wheezing seasonally which is making her show the symptoms which she is having now. This is not infection, no point in using antibiotics time and again. This is allergy and not infection.

Suggestions:

1. There are certainly alternative management therapies in allopathy now-a-days. Medicine has advanced a lot and wheezing is 100% controllable.

2. Inhalers are the newest management strategies for this. If I were your paediatrician I would have suggested the use of Budecort metered dose inhaler (100mcg) 2 puffs twice a day through a spacer and this is for regular use for 8 weeks. Another metered dose inhaler is Levolin and this can used as rescue therapy whenever the kid is having severe cough in spite of regular usage of Budecort.

3. The technique is very important and very crucial for the drug to be delivered correctly to the lungs. Regularity of medicine usage also matters a lot. So do not discontinue abruptly after you notice some improvement. The technique of administering an inhaler using a spacer has to be taught to you by your doctor and these are prescription medicines. So I suggest you consult your pediatrician for this.

4. Triggers can be environmental changes/ dust/ talcum powder/ seasonal changes/ un-cleaned a/c vents/ cold weather etc....we can specifically say this is the cause - unless we observe the kid closely - best person is the parent.

5. As for the betterment of acute symptoms now, I feel that she may require oral Prednisolone short course for a period of 3 days. This is a prescription drug and I suggest you seek your pediatrician regarding this.

hello doctor..yesterday I visited my pedestrian..she has giver her hydrocortisone 10mg 2ml injection and asked for Asthalin n budocart nebuliser of interval of 4 hrs . montair.lc 5mg once a day n kidpread 8ml thrice a day. .she is better then yesterday but not recovered n having weekness. planing to visit again pedtiric n ask doctor to give one more injection for better improvement.. is it safe to give one more injection of hydrocortisone ? .thank you doctor for your comments..please let me know what are the things I should practice to avoid instances like this...

As the kid is already taking oral kidpred - no need for hydrocortisone inj

Hi...as the kid is already on oral prednisolone (steroid) in the form of Kidpred syrup, no need to give Injection HHydrocortisone again which is again a steroid.

Continue the rest of the treatment as advised by your pediatrician.

You can avoid the triggers like - house dust mite, dust, cobwebs, well clothed and protected in chill weather and in seasonal changes, environmental pollution, vehicular smoke, cockroaches at home, cover the pillow covers with plastic covers, change the bed linen weekly, keep the child away while you are cleaning or painting the house etc.
