Suggest Treatment For Constant Diarrhea

Question: I have had diarrhea for about a week. I haven't been able to eat. I have been told that I am gluten and dairy intolerant. My symptoms always seem worse after my period. Went to urgent care yesterday and some labs, potassium, creatinine, bun were within normal range.
Any suggestions?

Based on history you have shared, it can be due to a variety of causes, as if there had not been any association with gluten, lactulose or periods we would have easily thought of gastroenteritis but as in the presence of all these it needs to be evaluated on all these grounds. Good signs are that your current labs are normal and that means that there isn't chronic electrolytes loss or dehydration.

Assciation of diarrhea with periods can be explained by increased prostaglandin during periods, increased smooth muscle movements or with hormonal changes whether during normal physiological response or in the setting of endometriosis and the later may need, ultrasound pelvis. Using loperamide in such circumstances will decrease gastric motility.

In the presence of lactulose or gluten intolerance, dietary restrictions must be observed. If you are not following those precautions, diarhea can be due to that. You need to make a systematic diet plan after meeting a nutritionist and continue using supplements for calcium, iron, vitamin D3 and B12.

Meanwhile treat as acute gastroenteritis with metronidazole for 5 days and anti spasmodic drugs like drotaverine or hyoscyamine, and send stool for examination and cultures. Also keep using oral rehydration salts to replenish list electrolytes and water reserves.
