Suggest Treatment For Constipation And Frequent Urination In Infants

Question: My 7 weeks old daughter started throwing up 2 days back. She vomitted 15 times. Doctor gave her domastal, bifilac and nexpro junior. The frequency has decreased and she has not vomitted in last 24 hrs (touch wood!)

However she has not passed stool in last 15 hours.she has passed urine 5 times in that period. Normaly she would poop 5 to 7 times and urinate 7 to 10 times in a day. Should we be concerned? Her stool had changed color from yellow to pale yellow and green in last two days. The stool test did not show anything serious except presence of mucus and lymphocites (border line) and some occult blood

It is good to hear that the vomiting has been controlled. But if it recurrs, you would need to consult a doctor to evaluate for a condition called hypertropic pyloric stenosis. If the vomiting does not recur, you need not simply worry about it.

For a child of this age, passing stool 10 times a day is as normal as passing stool once in 10 days and in neither case you need to worry. I would not worry about the stool frequency or its color. Rather, the frequency of passage of urine is more important. If the baby passes urine 6 or more times over a period of 24 hours, things can be taken to be fine.

The number of lymphocytes or pus cells in stool indicates that there is no bacterial infection. But the occult blood concerns me. I would suggest you to keep the baby in follow up of your doctor and get her reassessed subsequently.

Just let things continue as they are.

hypertropic pyloric stenosis would be caught in ultrasound scans,right doctor?we did get ultrasound done and everything is normal per report.

What could cause occult blood?

It should have shown up in the ultrasound had it been present.

Yes, hypertrophic pyloric stenosis should show up ultrasound. Since it has not shown up in the scan reports, you need not worry about the same.

Generally infections are the cause of occult blood in stool but it seems less likely with respect to the pus cell count. Surgical causes like intestinal obstruction could have been a cause but it should have also shown up in the ultrasound if such a condition was present. Malabsorption syndromes could also be a cause but I would not like to jump to conclusions right now and hence I suggested a subsequent follow up.

Thanks doc. For now my doctor has just prescribed above said medicines and said the mother to stay away from milk or any dairy product for a week. He has also given taxim for baby, but said it should be used only if after a week condition does not improve. Fingers crossed. Everything looks fine now, thanks so much doctor.

Your doctor is thinking on the same lines as mine. In case of lactose intolerance, milk and dairy products could not be digested and malabsorption might occur and give rise to occult blood in stool. Taxim O is an antibiotic and should take care of an infection, if present. If you start it, please complete its course. But since things seem fine as of now, nothing needs to be done.
