Suggest Treatment For Cough In A Infant

Question: Respected Sir,
My Child male baby 6 months 20 days has suffered Cough (Very High) due to his brothers (3 years 3 months)current situation the details given below:-
My child 3 years 3 months old has suffering Sever cough & Fever 20 days i was consult our regular pediatrician ( D C H) give medicines 24/2/2015 the following:-
1)OTRINOZ PEDIATRIC NASAL DROPS,2) DOLO,3)K-ZINE SYP,4)PROWEL DRY SYP,5)LEVOLIN SYP,6)COOL TOUCH GELPAD-CHILD,7)ASTYMIN LIQID,8)NEMOCID SUSP
again consult 06-03-2015 the following medicine given:-
1) Refzil 0 125mg syp 5 ml BD (9 days), 2) Asthakind Exp 3.5ml TDS(9days),3) Bifilac dry syp 5 Bd(9days), 4) Dolo 5ml (sos)
but the cough is still & fever every day evenging 3 days there then doctor suggest take Blood test, CBC,ESR,CRP,MX and result shows follows:-
HB 11.6GM, HAEMATOCRIT 33.5%, TOTAL RBC COUNT 4.43 millions, MCV -76 gl, MCH - 26.2 pg, MCHC - 34.7% Total WBC count 12200 cells, Platelet count 299000, Neutrophils - 53%,Lymphocytes 34%, Monocytes 4%, Eosinophils - 8%, Basophils 1% ESR(1hr) 90mm, CRP(C-Reactive protein) 5 mg, Mantoux test - Negative
Again he prescribe the following medicine:-
Singulair 4 mg Tab evening 30days
TECZINE syp 2.5ml bd
Hetrazan 100 mg tab 1/2 tds
vomikind sys (sos)
zenflox Eye drops
Now my question is my childs cough reduced but not completely fever also reduced some times it comes he was going play school 2 hours daily last 5 days what i can do now? how do reduce my childs ESR 90 mm
Now My 2nd child (6 months 20days) has also cough(High) the doctor medicine given the following?

Ambrodil -s syp 2.5ml tds
Zithrox 100mg Sys 3.5ml od
HICET syp 1.25 ml od

My question is shall i take further investigation,blood test, urine test etc now cough slightly reduced

I am sorry to hear that your son is sick. I have gone carefully through the details as well as the reports. Such symptoms are usually caused due to certain viral infections. Generally they are associated with an allergic component where cetzine (cetirizine, an antiallergic) is helpful. Antibiotics are unlikely to help as they are ineffective in the treatment of viral infections. Irrespective of the use of antibiotics, such infections may last for 5-10 days. Viral infections generally do not reflect in the blood tests. The fever is needs to be controlled by the use of paracetamol, and tepid sponging. The mininum interval between doses for paracetamol is 3-4 hours. Giving steam inhalation or use of humidifiers should be helpful by helping in diluting the cough. Just sit with the baby on your lap and place the source of steam before you. The water vapour will be in the air and automatically the child will breathe it in.

Although the medicines are ok to be given, I feel that the child is being overtreated. Irrespective of the measures, it will last for 5-10 days. Please be patient. But if the child grows lethargic or develops wheezing, you would have to take her to a pediatrician without delay. Otherwise I would not really worry.

Rather than giving Hetrazan, I would perhaps have tried deworming first. Deworming with albendazole syrup 10 ml single dose may be repeated at yearly intervals in children of this age.

For subsequent episodes of illness, I would insist on visiting a different pediatrician.

All the things that I said, apply to your second son as well. The medicines are proper. Be patient this is what I meant by not being proactive in my earlier reply. You do not need tests immediately. It is most likely the same viral infection that your other son is suffering from. It is unlikely to bring changes in blood reports. In case of unresolving and lasting fever for more than 7 days since start, you may get a urine test done to rule out urinary tract infection which could be a cause of the symptoms. But unless the child becomes lethargic, I would not really get concerned. If the fever lasts for more than 7 days, you would need to get the blood tested to rule out typhoid.

The cough is reduced no fever but stool passing is liquid with sputum for my second child

all the above medicine is over today, shall i continue any new medicine for my second child sir

For my first sons antibiotics are over sunday itself shall i continue any new medicine he has also some cough during food taken time he suffer little breath problem now that also reduce no fever, body temp 98.7 F always. Shall is continue (Nemocid) medicine?

Thanks for writing back. Sorry for not being able to write back earlier. If you have given 3-4 days of Hicet or Ambrodil-S, you need not give it further. Prowel surup can be stopped. I would suggest you to give plenty of curd instead. Zithrox needs to be given for a total of 5 days. Continue to give paracetamol if there is fever above 100°F. Nemocid (Pyrantel pamoate) is usually prescribed as a single dose. Only one dose of it is sufficient. It does not need to be continued. Give steam inhalation to both your children. It will also provide relief for the breathlessness of your first child.

I would not worry about the loose mucusy stool of your child. It could be due to the antibiotic getting rid of the beneficial bacteria as well. Curd will be helpful in this regard. Give ORS (oral rehydration solution). It will replenish for the salts and water lost.
