Question: My right great toe has cartilage damage due to trauma (sliding directly into a shower base OK a slick tile floor multiple times. X-rays show that the toe's "knuckle" bones are not cushioned and are in direct contact with each other.

Is replacement of the damaged cartilage with silicone gel a possibility or is some other replacement , such as a donor m meniscus, a better option?

The damaged cartilage of the big toe is difficult to repair surgically. The replacement of the great toe is usually recommended in case of a severe arthritis and cartilage damage.
Replacement with silicone gel can help but metallic resurfacing of the joint may also be needed to smooth en the surface of the bone. Donor meniscus is not tried in great toe due to lesser chances of acceptability and a complicated surgery due to a small joint.
It is better to consult your surgeon and consider metallic resurfacing of the joint along with silicone gel cartilage.
