Suggest Treatment For Dark Circles Under Eyes And Upset Stomach In Child

Question: Four year old has very dark circles under his eyes.
Grandchild had stomach upset for 24 hours, couldn't hold anything down. Parents gave him Tylenol and tepid baths.

Thanks for the query

If the dark circle under the eyes are recent finding then in presence of loose motion it is most likely due to dehydration.
If hydration is not managed properly in case of fever and loose motion there is a high chance of dehydration.
You have mentioned in the medical history that he also has a murmur.
I assume this murmur is due to some congenital acyanotic heart disease (most likely VSD).
Fluid management needs to be very precise if there is loose stool in a child with heart disease.
Fever can cause increase load on a compromised heart and thus can precipitate heart failure.

I suggest you to give tylenol in a dose of 15 mg/kg/dose every 4-6 hourly.
Do tepid sponging.Give plenty of fluid orally to combat dehydration.
If he is on any diuretic (for heart disease)stop it immediately as this precipitates dehydration by increasing urine output.
Most of the time the fever and loose motion in this age is due to some viral infection and the main aim is to prevent dehydration.
If there is decreased urine output(difficult to assess if child is on diuretic),oral acceptance is poor,baby is getting lethargic or irritable all these are suggestive of dehydration.
I would like to get him evaluated by a pediatrician on urgent basis (as there is also heart disease).
