Posted on Mon, 6 Oct 2014
Question: My wife is suffering from depression, extreme anxiety, fibromylgia, body pain, some time high pulse rate from last 3/4 years. Recently she also starting having high bp. Dr. has now changed her medicine to Betacap tr 40 (one a day), Retense 100mg (twice a day), Lonazep .5mg (Thrice or Twice a day), Newtel H 40 (Once a day) and Milza 25mg (Once a day). From last 7/8 days, my wife feeling very weak and unable to walk even for 4/5 steps. She feels like no energy at all. Also from last 1 week she gets severe anxiety attacks twice/thrice in a day after which she looses her sense for few minutes and then comes to normal slowly. Kindly suggest if her treatment line is ok. Does yoga, meditation helps in this situation? Is using Milza 25 is safe for use as I do not find much reviews on this medicine in internet.

Brief Answer:
stop or reduce retense & betacap. take bp

Detailed Answer:
Hello,
Milza is fine as it will help in both pain of fibromylagia and treat depression plus anxiety.

rretense should be avoided if possible as it can give rise to anxiety attacks and also dizziness. This also has major risk of hepatotoxicity.

newtel H is a good molecuel for hypertension managment and can be continued.

betacap is usually not given in such high dose espically in female patient who are already on antihypertensive meds. Most possibly the lightheadedness or falls are due to this drug. This also give rise to ghabrahat or anxiety in many cases.

You can get your wife's BP cheaked twice a day for one week and see if there is a fall or rise in specific time of the day. This can be discussed with her doctor and he can manage the bp in a better way.

I usually try to give betacap in a dose of 5-10mg in morning and afternoon for some days and then taper and stop it. This is usually not needed for managing anxiety or BP as there are many better molecules available.

So, you may discuss the points with her treating team and see if it works. At least with less number of drugs you can come to know what is working and what is not.

Yes, diet and exercise hace a good role to play in any patient espically of chronic pain condition and anxiety.

you may start with light walk for 15-30min in evening or morning initially and then once she is ok with this you can proceed with jogging.

regular gymming also helps a lot. One should ideally go for 3-4 times a week and not daily.

She may try deep breathing exercise twice a day. But this should be learned properly.

include flaxseeds( powder) in her diet daily. Ask her to take more of salads and sprouts compared to carbohydrates.

She should definitey move out once a day for an hour or so . Going in a park or socializing is one of the best methods available in reducing axiety.

she might resisit initially but when drugs start showing some result -she will agree.

Hope the reply is useful.
please feel free to ask more queries for clarification.
