Suggest Treatment For Depression Inspite Of Having Bipolar Disorder Treatment

Question: I need another medication to help me through this depressive downswing of my bipolar II disorder. I already take cymbalta and abilify but i need something more potent to take when i feel the urge to commit suicide so i can calm down. I have xanax 1mg but it doesnt help during those times.

Hello,
looking at the question I feel that you should be on some more potent antideressant that has a good anti anxiety action as well.
Drugs like paroxetine and venalafaxine will be more suitable to you compared to symbalta. Symbalta is OK but a mild drug and cannot give good results in your case.
You may talk to your treating team to shift you to any of the above two choices.
There is no need to worry as, many times the depressive phases need something strong to tide over and reduce the crises episodes.

The good anti depressant will prevent occurance of such episodes and prevent the tendency of mind to slip into suicidality.

For Paroxetine I got ringing in my ears and had to discontinue it. Is there anything else you would recommend?

venalafaxine, sertraline or amitryptaline

Hello,
not a problem, there are still many good choices left as you are young and there are no comorbid illnesses. You may take low doae (37.5-75mg) venalafaxine as mentioned. It is a very good molecule and the effect usually comes in 3-4 days.
Apart from venalafaxine you may try amitryptaline or even sertraline. Both are effective in controlling your problem.
Sertraline is very safe and can be given to even cardiac patient.

However, once you have good results with a different drug, please discontinue symbalta as, there is no point in taking two drugs for one purpose.

Also get your TSH and Sr. B 12 levels done.
