Suggest Treatment For Depression

Question: Hi, Two years ago, at the age of 22, after a long struggle with depression (since childhood) I finally sought medical help and went to see a counsellor who sent me to psychologist. I was diagnosed with clinical depression and given nexito 20mg prescription along with CBT by that doc. I was not able to take up the CBT due to having too much college work. I did take the meds, and stopped going to my sessions with the doctors as I felt I was getting better. On my own, one day (after six or seven months of taking the meds) I stopped taking them without consulting the doctor. It has been one and a half years since and I have been doing pretty good. But now, I'm facing a lot of stress and setbacks in life (nothing major but still) and it is really effecting my mood. For the past few months I have had mood swings, and now they are becoming more serious day by day. I feel very depressed and negative at night, and cry for hours. I am very insecure and doubt myself and my abilities all the time. I start thinking I need to see a shrink again. But the very next morning though, I feel completely okay, even great. I feel like I can conquer the world and I feel a huge surge of energy even though I have barely slept. I feel like I don't need any doc or meds and with excercise and keeping myself busy with my new projects I can be happy. Things that made me feel so depressed the night before hardly seem to matter now (such as a loved one moving to a different country, failure to get the job I want). I just need suggestions on what I should do, think or believe in? Which part of me is the real, reasonable person??? HELP!

Hello,
The most important thing here is - both low and high part you are feeling is "yourself"....the low part is due to depression and high part next day is mania.
You have most probably developed bipolar disorder now. This is a type of psychiatric illness in which - one time (this phase may last for few hours to many months) he feel low/off/lethargic/reduced confidence, etc and in next phase (whose duration may again vary greatly similar to previous one) characterised by high energy level, positivity/ increase working capacity/wishfullness etc., and elated mood etc.

You should start a mood stabiliser like- lithium or sodium valporate or oxcarbazepine.
Get some blood tests- CBC, TSH, lipid profile, RFT and LFT.

As you have mentioned, these fluctuations (mood swings) have become severe now-I want to inform you that they may further increase in intensity and distrupt the daily functioning and even your relations.

With meds you will start feeling better in few days only.

Please see a psychiatrist and discuss the above points.
