Question: I wanted to ask you about my daughter. She is 49. She has MS and diabetes. She has been having left upper abd pain. Thdr's so far ruled out any Gi problems with an EGD and colonoscopy. she had blood work done for pancreas problems. All Were normal. She had her gall bladder out about 15 years ago,
This has been going on for about 2 months. Today she says her lymph nodes in her armpits are painful and swollen. She is running a lw grade fever and is feeling ill. Achey, headache, ear ringing etc
Could you tell me what may be going on.

Thanks for your query.
Noted the history related to your daughter F/49 with MS and diabetes - undiagnosed left upper abdominal pain - 2 months - LNs in armpits are enlarged and painful - low grade fever and has been achy, have ear ringing and headache...
The probable cause of recent development can be:
Infection in the lymph nodes giving the said symptoms. The cause can be diagnosed by investigations alone. If the lymph nodes are symptomatic in both the armpits, the causes can be infection in general or rarely can be infection or such things in both the upper limbs or breasts - in the draining area of armpits.
I would advise her the following:
Tests of blood: CBC, HbA1c and sugars, blood culture and sensitivity helps sometimes and other relevant tests as per the clinical evaluation.
Ultrasound of the armpits and FNAC of the enlarged lymph nodes under ultrasound guidance.
Clinical evaluation and examination is very important to get the exact picture, to have physical examination of upper limbs, breasts, abdomen and further investigations as per the clinical findings.

Please give additional information:
Is the pain in the upper abdomen actually on the lowest ribs suggestive of costochondritis or in the abdomen suggestive of colitis?
Any symptoms related to bowels or urinary tract?
Please give details about the pain in abdomen, may help for probable diagnosis.
