Question: Is there any cure for Disseminated Superficial Actinic Porokeratosis? I have it on my legs and it is driving me insane

I have nomore questions, yes a dermatologist diagonosed the DSAD. I am having troble sending pictures..just the rum of the mill DSAD..I have been seeing Dr. for at least 2 years or I do not see the Dr. I only see his assistant.. no real medications for the DSAD only things to help with the itching..I know I'm not the only person in this world with this problem so someone must have some answers!

Welcome again and thanks for your follow up.

I am highly empathetic to your skin condition and can understand your part of sufferings. It is quite fine if you can not send the pictures. It was to confirm the condition as disseminated superficial actinic porokeratosis (DSAP).

Now presuming your condition as DSAP based on the diagnosis given by your dermatologist there are certain important things to guide you:

-     Porokeratosis is a skin condition which starts as rough bump (we call it as a hyperkeratotic papule) and then it forms a flat surface raised structure( call it as a plaque). There is typically an annular appearance due to its thread-like elevated border that expands centrifugally (peripherally).

-     The disorder is named porokeratosis because the microscopic appearance observed (we take a skin tissue and analyze it under a microscope).

- At least five types of porokeratosis have been recognized and amongst these disseminated superficial actinic porokeratosis (DSAP) is one of the most common one (which you are having).

- It appears on sun-exposed areas of white-skinned individuals, becomes more prominent in summer and may improve in winter. SO the most important of the part in prevention of the problem is by sun avoidance.

In my patients with your kind of problem I always guide my patients to have strict sun protection. This is sole most factors in further aggravation of the condition.

For that I suggest my patients to wear full sleeves cotton clothes. Using a hat during day time and also a sunscreen everyday on exposed areas.


There are different modes of therapies for the treatment of the DSAP. In my patients I prefer a wide range of procedures depending upon the clinical condition of the patients.

It includes Cryotherapy (using a liquid cold gas to ablate the area) , topical 5-fluorouracil (5-FU) cream , topical retinoids cream incombination with 5-FU and topical imiquimod creams.

Sometimes I suggest my patients to go for laser (such as CO 2 and other lasers) and even surgical procedures such as shave excision, curettage, linear excision, and dermabrasion.

I suggest you to meet a proper dermatologist and get yourself examined properly so that most appropriate treatment can be started. You can discuss various treatment options enumerated above with the treating dermatologist.
