Question: I just got off citalopram 40 mg...my primary doctor is not an expert in the 'bipolar' field, however, I have not discussed much of my issues with him. I have been sleeping for at least 12-18 hours straight lately and can only make it to work about 2-3 days a week (luckily I have a job where this is not noticed). From my research, I believe I am definitely bipolar. Just not sure how to proceed, don't really want to deal with my primary doctor on this and was thinking about going to the local mental health clinic

I can understand your concern. Bipolar disorder is a mood disorder in which individuals have extremes of mood either symptoms of mania or hypomania alone or depression alternating with mania. The mood episodes varies between depressive disorder and manic disorder. In between episodes individuals remain symptom free.

You have been on Citalopram and the drug is a SSRI group of antidepressant. The symptoms you are currently having like increased sleeping, inability to work properly are not the classical symptoms of bipolar. If you could elaborate the symptoms then we can proceed to make diagnosis of bipolar disorder. I'd like to know in follow up if you have any of these symptoms presents or in past-

- Persistent anger outbursts or irritability.
- Persistent euphoric, cheerful mood as noticed by others
- Feelings that you have increased energy
- Ideas that you have some super powers or grandiose ideas
- Increased money spending even on minor things
- Increased sexual interest, increased sociality
- Increased risk taking behaviour
- Inability to concentrate on job because of distractibility
- Decreased need for sleep etc.

These symptoms are commonly seen in mania. Mania is one of important pillar in diagnosis of bipolar disorders. Please provide me details in follow up question so that we can proceed further. Also provide me details about your current symptoms, how they started and any aggravating or precipitating factors.

Yes you may go to the local mental health clinic and discuss your issues with the doctor there.
