Suggest Treatment For Dry Cracked Skin On Fingers

Question: some skin allergy or infection problem and the problem is on two left side finger. It cures after long 20 days or so and repeats again after some time. The skin cracks up and the skin opens too. It looks as if it has been cut by a blade and slowly closes after 5 to 6 days. During this process it pains in that area. The problem is persisting since 4 months. The area is slowly increasing. It is on the inside portion of the finger (palm).sometimes itching starts on the back, joints, groins and specifically at the inside face of folding positions of arm, leg etc. I scratch a lot on such positions including the area around testis and it will get stopped after sometime. the rashes will also fade away in a day. Itching will be even on my head, around neck. If the atmosphere is very hot and the sweating is much the same will increase. Due to my itching on the head I lost much hair due to the same. I thought the same will correct by itself but is persisting. This is like intermittent. Please advice

I understand from your query that you are having 2 types of problems-
1. First one is the itchy cracks and fissures on your hand

2. Second is the itching on the body folds and elsewhere intermittently

Let me address each one.

1. I have seen the picture attached by you. The possibilities of the scaly patches with fissures on your palms could be Hand eczema ( allergy) or Palmar psoriasis.
Eczema occurs due to an external allergen( e.g- Touching plants, specific foods to which you may be allergic to, latex, rubber,etc.) or due to an internal factors ( in people prone for allergies form childhood like nasal allergies, asthma, etc).

Have you been in contact with any of the above mentioned or similar allergens? Since it is of a recent onset, please sit and think of all possible contactants in the last 4 months ( like new rubber handle on car steering wheel/ gear box,etc) which could be triggering the allergy.

Palmar psoriasis is a type of skin problem usually seen on both palms. So I would consider it as a less possible cause.

You could apply a cream containing Clobetasol propionate and salicylic acid combination twice a day for 2 weeks. This should be followed by a regular use of a thick moisturizing cream containing XXXXXXX butter, cocoa butter, pertrolatum,etc.

Please get the cream prescribed by your dermatologist.

Eliminate allergens if you can thing of any.

2. The second problem where you have itching in the folds is called as intertrigo. This could occur due to overweight and constant friction in the folds. This is also more common in humid weather.

Use a powder with 1 % Clotrimazole twice a day on the areas. Keep the areas as dry as possible.

Regarding the 1st - Yes, I came in contact with Fevi quick while doing some application at home it stuck to these fingers and ultimately it led to this problem. Else I do not have problem. Please prescribe the exact medicine so that I can start applying.

Regarding the 2nd - As explained earlier I am itching not only on these foldable areas, near testis but also on the head, around neck and on my back i.e. the larger plain area, compete arms, legs, thigh portion etc. which are open too. So kindly suggest a medicine for this too.

Please prescribe the exact medicines for these two problems, so that I can take start taking these medicines immediately. As described by you I am Asthmatic.

Hello,

1. The allergy could be due to feviquick as per your history. Sometimes, if you are prone to allergies, once it gets triggered by an external agent, it can persist even after the contact.

2. As far as the generalized itching is concerned, I could be a part of your allergic skin nature. ( since you have history of asthma). We call this as " Atopy". But considering your age, I would also consider dry skin as another reason. Usually, with increasing age, the skin lipids reduce and can cause generalized itching especially in people with Atopy.

Other causes for generalized itching could be medicines( which you might have taken recently), liver disorders and thyroid problem.

I would suggest that you use a soap free cleanser like Cetaphil skin cleanser for bathing instead of soap as soap strips away the moisture from the skin.

Use a moisturizer twice a day all over the body.

If the itching persists 2 weeks after using the above, please get a blood test to check thyroid function, liver function and hemoglobin to rule out internal disorders.
