Hello. Thank you for writing to us.
I have taken note of your query and I have also reviewed the Image
This is Hyperkeratotic Eczema and I would suggest a skin biopsy to rule out palmoplantar psoriasis.
This is not fungal infection.
You must use a moisturizer regularly, preferably an ammonium lactate Or urea based moisturizer.
In addition I suggest you to use a potent topical steroid e.g clobetasol propionate 0.05% with salicylic acid 6% combination ointment, twice daily, over and above the moisturizer.
Oral retinoids e.g Acitretin can also be started.
This would require a few blood tests like Liver function tests and Serum Lipid profile at baseline and at regular intervals.
