Suggest Treatment For Dysentery And Regular Bowel Movements

Question: I am a female, 29 years old. I have been suffering from dysentery on and off for the past ten years now. I don't have regular bowel movements. Sometimes it is loose motions and sometimes it is constipation. I have got my stool tested two years back and amebiasis was found. I use Norflox-TZ for five days, the bowel movement gets controlled for a few days but I get back to the irregularity. Tried Norflox-TZ many times, but no respite. Advice.

Dysentery refers to blood in stool. If that is the case, it is more likely to be caused due to certain bacteria although amoebiasis is seldom the cause. But I am not sure whether you mean diarrhoea or dysentery. Recurrent diarrhoea or dysentery is most often due to certain malabsorption syndromes and conditions like irritable bowel syndrome, gluten enteropathy and certain other related conditions. It is less likely to be due to infective cause and hence no response to Norflox-TZ is expected.

The things I would suggest are:
* Take a high fibre diet. Take plenty of green leafy vegetables. Fibre supplements like isabgol and senna may be taken on regular basis.
* Avoid ALL wheat based products for two or three weeks and see if it makes a difference. If it does, chances are that you are gluten sensitive.
* Regular morning walk and a healthy lifestyle is essential.
* It might be worthwhile to get a consultation from a gastroenterologist.

Dear Sir,

Yes, it was diarrhoea and not dysentery. I didn't know the difference before.

Along with wheat products, do I need to stop the Maida products too? Can gluten-sensitivity be developed at this age too?

I have PCOS and I am over-weight. Does it have any relation to what I am going through? Also, I have been away from home for a long time (hostel life) and eating out is something I frequently do.

Please suggest me a medicine for this malabsorption syndrome for the time being as it takes time for me a consult a gastroenterologist. (I am now in my job training)

Wheat based products include maida, suji along with bulgar wheat, durum wheat, wheat XXXXXXX wheat rusk, wheat flour, wheat starch, modified wheat starch, wheat protein, rusk, semolina, couscous, barley, barley malt, barley flour, rye, rye flour, triticale, kamut, spelt. Buckwheat, corn, malted breakfast cereals, oats not labelled as gluten free. They need to be stopped too. Try this for 2-3 weeks and depending on whether there is any improvement it can be determined whether you have sensitivity to these. I understand that it might be hard in case of a hostel based life. Try a rice based diet instead.

In case of malabsorption syndrome, high fibre diet is the main thing to suggest. Try it and see the response. It is only based on that you can schedule the appointment with a gastroenterologist. I do not think that you need to take any other medication as of now.

I do not think that it is related to your PCOS.

Dear Doctor,

If it is less likely of infection why do I have an immediate relief which stays for 10-15 days when I take Norflox-TZ ? Everytime I take it, I find relief. However it doesn't stay for more than a month and I am back to the irregular bowel movements. I just wanted to know this before closing the discussion.

Thanks for writing back. Sorry for not being able to write back earlier.

The mentioned medicine is an antibiotic. It so happens that the human gut is not sterile and populated by tons of bacteria. In fact many of these bacteria are beneficial too. Norflox TZ clears the normal bacterial flora temporarily and this may be associated with some relief of the intestinal condition you have, even though it is not directly associated with bacterial infection. But soon the bacteria return and the condition goes back to the starting point.
