Suggest Treatment For Elevated Blood Cholesterol Levels

Question : I was diagnosed with fatty liver about 2 months back and was prescribed Udiliv 300 and Evion 400 BD. Only my SGPT was elevated to 114 while all other enzyme were in normal range. My cholesterol levels were slightly eleveted to 202 while my LDL was 147. For two months I controlled my diet and lost nearly 5 kgs. I again did the test yesterday, all my liver enzymes have returned normal but my cholesterol has shoot up to 232 with LDL reaching 174. My triglycerides levels have also increased. I was worried as to why is my levels increasing even after weight loss and controlling diet. My intial weight was 74 and now it is 69. I am male aged 30 and height 168 cms.

Hi,
welcome to HCM,

I have read your query and understand the concern,

First,I would like to say that 114 SGPT level does not have any clinical significance especially if your other liver function tests are normal and you do not have clinical symptoms. Fatty liver might be the underlying cause but it is not a matter of concern here since fatty liver is completely reversible with proper dietary habits and exercises.

Now coming to your lipids or cholesterol part:

One may experience initial slight elevation of blood cholesterol and triglycerises or may slight reduction in the HDL (good) choleserol as this could be because of multiple factors like,

i) Because of dieting and exercises,our excess stored fats (source of energy) mobilizes from fatty tissue into the blood stream,so you may see slight rise in cholesterol level at initial stages. (we may expect it to come down later on).

ii) It could be due to laboratory variations

iii) It also depends on the type blood sample (fasting or random),time of giving sample (morning or evening) and other bodily factors.

Since you had seen cholesterol elevation from 202 to 232 with LDL level from 147 to 174 which are not significant clinically.

Also,I want to clarify that our blood cholesterol values keep get fluctuated in the blood just like our pulse rate,blood pressure and blood sugar.

So,there is no need to worry much about this mild elevation as it will come down to normal within 6 to 8 months.

You can continue your dieting with exercises and continue to take Udilive and Evion medications as suggested by your doctor.

Thank you Dr. XXXXXXX for the answer. Yes you pointed out rightly all of my lipids have increased and HDL has decreased a few points to 37 from 40. Sir, I wanted to ask a few additional questions:

Should I decrease the intensity of my workout? At present I jog for about 30-40 mins everyday and my diet control is medium. Should I need to increase dieting levels or activity levels?

What should I do to reduce my level of cholesterol?

Secondly, when should I get my test again to recheck my lipid profile?

Also, the doctor had asked me to get ANA test done when I was diagnosed with fatty liver, now that my liver enzymes have returned normal should i need to get ANA test.

Thank you once again for the help. I was relived to know that there is nothing serious.

Hi,
Welcome again and thanks for the follow-up,

There is no need to increase the intensity or time of workouts,you can do it as per your convenience. You can continue with your dieting as you have reduced weight since couple of months.

To reduce your bad Cholesterol and to increase good (HDL) cholesterol follow these dietary guidelines along with regular exercises.

i) Avoid direct sugars intake and fried snacks, bakery items,

ii) Take good amount of vegetable salads- raw vegetables like cucumber, tomato, carrot and vegetables.You can also eat them raw.

iii) Take healthy snacks or nuts like almonds or walnuts and a few dry fruits but no fruit juices,

iv) Avoid soft drinks as they are only for taste having NO nutritional values.

v) Take vitamin B3 (niacin) as it is believed to increase HDL cholesterol.

You can go for yearly check up of your lipid profile and there is no need to worry about this mild high cholesterol.

In my opinion,there is also no need to go for ANA test since your liver enzymes have come back to normal but still I would suggest you to follow your doctor's instructions as he knows your condition better.
