Brief Answer:
needs investigation, ?Multiple goitre ?Nodules

Detailed Answer:
Dear,
Welcome to HCM.
As I can understand your chief complaint is of thyroid swelling.

The condition suspected here are multiple goitre, thyroid nodules, benign or malignant.

I would need to have information regarding following
1) Tenderness or pain in the front of the neck, Hoarseness,Cough that is new and persistent, Coughing up blood, Shortness of breath, difficulty in swallowing?
2) Bone pain in one spot without relief
3) Weight loss, heat intolerance, trembling hands, palpitations, insomnia, anxiety, increased bowel movement frequency - especially if the symptoms are new or persistent
4) Weight gain, cold intolerance, constipation, very dry skin, slowed thinking, depressed mood, muscle cramps.
5) Family history of thyroid cancer

answers regarding these can provide a better clinical picture.

My advice for you:
1) visit a local surgeon, as a detailed palpatory examination is needed to confirm the condition.
2) investigations of Sonography, MRI, Fine needle Aspiration cytology, Thyroid function test measuring TSH,T3,T4 can be indicated to check of cytology, extent of spread, size, function of gland.
3) depending on the findings further treatment can be decided.

Hoped to have satisfied your query.
Welcome with details and for further suggestions.
Regards,
Dr Pankaj Borade
