Suggest Treatment For Enlarged Liver

Question: enlarged liver 20,0cm homogenous in echotexture,pancreas oval soft tissue masses seenin peripancreatic area and periheptic region largest 2.9x1.8cm,gallbladder ditended,rt kidney homogeneous echogenic focus 1.5x1.5cm,splenic volume is 5. lymphadenopathy suggested in peripancreatic region and porta heptis....please help me make sense of all of this

-- There are some masses around the liver and around the pancreas. The way how they are described seems like, they may be lymph-nodes which are for sure enlarged. Lymphnodes generally get bigger in size due to some infection, blood disorders, malignancy, but it doesn't talk about a specific malignancy here.

-- The spleen seems very small. But I'm not sure if you have given me the right numbers, or if there's any other data missing, because 5 is a very small spleen.

To determine what causes these enlargement you will need to follow up with your doctors and do further needed tests.

Thrombocytopenia with bleeding gums and nose bleeds, history of smoking. incidental note is made of pericardial effusion and bilateral trace pleural effusion trace rt upper quadrant peritoneal ascites.6 gallbladder distended no stones,thickness,or fluid is appreciated bile duct is normal (0.6cm). Spleen the spleen volume index is 5(normal8-36) aorta the abdominal aorta is normal. Blood work-high: ast36,globulin5.0,protein8.6, protein total 8.9, rdw17.9, low-albumin/globulin ratio 0.7,ferritin 14,large platelets few, iron and iron binding cap- % saturation 11, iron total 34, platelets 125,metamyelocytes 1.0, target and tear drop cells few, 1+poikilocytosis 1+polychromatic,1+anisocytosis, There's nothing more to add this is all the information I have maybe the added stuff will help you help me thank you

Hello,

Thank you for the follow up question and the extra provided information.

What else I read from your description here is :

-- You have low platelets in the blood which cause the bleeding. This may be due to the damaged liver function.

-- The liver seems dysfunctional because the fluid in the abdomen is a sign of that.

-- You have anemia. The cause of anemia needs to be found and get treated accordingly.

-- I think that there may be some hematological abnormality (further investigations may be bone marrow aspiration and cytology for immunohistochemistry etc. needed to comment specifically on this) or some infection or a tumor or infection , something that have damaged the liver and has caused all these.

Ultimately, you will need to be admitted in the hospital and get treated for this thoroughly because of a complex nature of treatment and may need some time until you get completely better.

Now a days there are a lot of treatments available and there is possible cure for almost every other ailments. So please don't get discouraged but trust your doctors.
