Brief Answer:
as explained in details.

Detailed Answer:
Hi.
Thanks for your feedback.
Feeling something unusual in the left side of the scrotum and heaviness in the left side in the end of the day on the left side indicates that you have symptomatic Varicocele.
The Epididymal cyst on the right side is asymptomatic and looks to be an incidental finding. Since it is a simple cyst, can be left as it is. Obviously the fresh ultrasound will show what it is like now.
I would advise you the following with explanation:
- First of all get a fresh ultrasound with color doppler. This will give the present status and guide for further care.
- Semen analysis: varicocele increases the local temperature of the testis. For normal sperm production the temperature of the testis needs to be 2 degree Celsius less than the normal body temperature. Varicocele by retaining very large amount of blood around the testis raises the local temperature of the testis and hence affect the sperm production directly. Since you have varicocele for a long time it is possible that the semen analysis can show whether it is normal or have reduced sperm count and quality.
Such changes is one of the indications for corrective surgery.
- Role of Ayurvedic medicines: These are not well studied although many companies/products claim that they help. It can not reverse the process by which varicocele has occurred.
Hence, if the varicocele is symptomatic and has affected the sperm counts and quality, surgery is indicated.
Till then use scrotal support to have elevated testes.
Avoid hot atmosphere.
Cold bath by tap water to the scrotum helps by reducing local temperature.
- Get an opinion of an Urologist also, get fresh ultrasound and color doppler and semen analysis to have the most proper evaluation to decide the further course of actions.

I hope this answers all your queries.
Please feel free to ask for further relevant queries if you feel that there is a gap of communication.
