Welcome to HCM
As i can understand from your complaints that you are having problems with erection when with a female partner.
We are considering a condition of sexual dysfunction here.
However to establish one I need you to answer few questions
Do you feel desire to have sex?
You are able to get hard but not able to maintain erection for long, is it?
Can you masturbate satisfactorily?
How about orgasm related issue? are you able to reach climax satisfactorily?

Answer to these would help me to establish a clear problem.
Next we would try to get to the probable cause.

Kindly reply.
Regards.

I do desire sex. I can masturbate successfully. I notice if I am turned off visibly...its not going to happen. I may get hard but not as hard as it should be. I may get hard but fade quickly. If i take a cialis tablet, no problem after the waiting period. But i may have problems getting that same hard reaction, 4 hours later.

Brief Answer:
Very informative!

Detailed Answer:
Thanks for the information.
After understanding your compliant I could establish an erectile problem.
Certain conditions like diabetes, hypertension, medications, stress, psychological causes, can be considered.
As we have a vast list of differentials, I would still require some more information to narrow in on a specific condition. Only then I would be in a state to give you a comprehensive suggestion.
1) are you having diabetes, or hypertension relate issues? if any what medication are you on?
- diabetes in a long run can block blood vessels of penis and hence hamper erection. Moreover, it can damage peripheral nerves that can further add to the erectile problems.
- certain medications used for controlling hypertension can cause erectile dysfunction
Here I suggest- a work up of HB1AC levels, blood sugar testing can be require depending upon the clinical signs for diabetes. I suggest an opinion of a local physician can be helpful.
2) how are your daily activity levels? sedentary office work or moderate physical work?
- When the lifestyle is sedentary or less physical, the body ability to maintain endogenous steroid levels is low. Low endogenous steroid levels can compromise erectile functioning.
3) any formal physical work out or exercise, do you practice?
- A moderate amount of physical work out can enhance the mood and libido via secretion of endorphins. It can also help to reduce the stress levels.
4) how often you masturbate?
- Frequent masturabation can drain out the libido and desire to have natural sex. Here I can suggest to indulge more in natural sex than to masturabate. An opinion of sexologist or psychiatrist can prove very helpful.
5) how frequent do you use cialis?
- As we know any drug in frequent and inadverant use can produce tolerance and serious side effects. Therefore it is suggestible that use of such drugs has to be strictly on the ADVICE OF DOCTOR.
5) any addiction, smoking, alcohol use, etc?
- Lastly, dependence on any substance can damage the sympathetic system(important on erection). Therefore if suffering from any of these, an opinion of a psychiatrist is advisable.

Information on these can help me to be able to give you precise advice or suggestion.
However, in any case it is advisable to consult a local psychiatrist/sexologist, as nothing can substitute a in person consulation.
hope you find my answer helpful.
you can always revert back with more information and for further suggestions.

Dr Pankaj B
