Question : Hi doc..I am getting married to my girlfriend next week .In my childhood I got one of the testicle due to an accident on cycle.
My girlfriend used to have sex but whenever I used to ejaculate within 20-15 seconds.Next erection takes 15minutes that too last less than a minute.
Girlfriend enjoys sex but she is disappointed she says we have tried foreplay and oral but do not give a solution.

Any remedies for the same doc ?

Hi,
As you have posted, you are a young male with a problem of premature ejaculation and erectile dysfunction with a past history of accident and henceforth you are having one testicle.
However, my understanding is premature ejaculation is very commonly coupled with erectile dysfunction and they are rarely isolated problem. I don't think this can be linked to your accident because the problem of erectile dysfunction is mostly due to some problem in central nervous system control.
You will be surprised to know that the most common reason behind these problems are anxiety, stress and a difficult-going relationship. Considering that you have a healthy relationship, you need to introspect regarding any anxiety or stress and do some exercise and relaxation sessions for a good sex life. Additionally, increase oral intake of water is also needed.
You additionally need to consult a local urologist for detailed clinical examination and you may need the following investigations to rule out abnormalities including endocrine problems like diabetes, hypothyroidism, low testosterone level or any urogenital problem:
Complete blood count including ESR, fasting sugar, fasting TSH, serum testosterone (at 8 am), serum LH level, serum prolactin, serum electrolytes
Urine Routine Examination
Nocturnal penile tumescence testing
Duplex Ultrasound including evaluation of cavernous blood flow

Your future treatment depends on the findings of the investigations. Many of the conditions can be reversed with appropriate treatment. But, an early diagnosis is always important.
So, please don't be anxious and apprehensive. Exercise, hydrate yourself adequately, avoid vigorous masturbation or any other measure displayed in cheap advertisements, consult a urologist and do the above mentioned tests.
