Suggest Treatment For Excessive Bleeding During Periods And Uterine Fibroids

Question: Good morning Doctor/s
I am having the problem of excessive bleeding during periods with heavy blood clots, right from the birth of my only daughter-in 2000. I am also having 2mm fibroid in uterus, in a stable condition from 4 years. Doctors only suggested to use Tropic MF 3 tablets per day. Even-though it worked for some months - then the doctors advised to use 2 tablets of 500mg Tropic MF as single dose and use 6tablets per day,
Eventhough Blood clots and heavy bleeding is not completely stopped, my question is how far it is advisable to take 6 tabs per day for 4days for the coming 8 to 10 yrs or so.
Eventhough safe - is there any alternative for this problem - like copper T or removal uterus - as nobody is suggested as these for my problem.

Detailed Answer:
Hi XXXX, I have gone through your question and understand your concerns. The first line treatment for cyclical heavy menstrual bleeding is tranexemic acid and mefenamic acid. It can be maximum in a dose of 3900 mg/ day for maximum5 days in a month ( that is 6 tablets of 650 mg tablets in a day). There are side effects like disturbances in colour vision, ocular disturbances, hepatic and renal abnormalities when used for a very long time. It is not a recommended treatment for long term.
Moreover, a fibroid of 2 mm will rarely cause severe and excessive bleeding.
Since you are 41 years of age, I would suggest you to get an endometrial biopsy to rule out any endometrial pathology like hyperplasia or carcinoma, however it is a rare possibility as bleeding in these conditions is irregular.
I would suggest you to get a progesterone intrauterine copper device ( mirena) insertion as it is highly effective for controlling heavy menstrual bleeding and is for 5 years.
You can also go for minimal invasive surgery like endometrial ablation.
Removal of uterus is the last resort when all the above treatment fails.
