Question : Hello. My name is XXXX and I am 19 years old. Last Sunday I had the 24 hour virus and was constantly throwing up and it went on until about Tuesday. Every since then I've been experiencing extreme gas that hasn't been able to leave my body and servere and painful bloating. I have tried literally everything such as gas x, ginger, tea, water, probiotics, prebiotics, acid reducing pills, and I also got prescribed pills that are suppose to prevent my stomach from cramping. I've seen two different doctors and nothing has helped at all. I'm a very healthy and active person I always work out but this bloating is getting out of hand and I'm not sure what else to do.

I appreciate your concern

Treatment resistant dyspepsia requires further workup. Polycythemia , hypercalcemia , malabsorption and H. pylori manifestation/infection can cause such a presentation.

Empiric treatment with PPIs such as esomeprazole and prokinetic like itopride at full dose for 2 weeks should be attempted. H. pylori testing and eradication can be done to look for improvement. Offending drugs like NSAIDs, nicotine etc should also be looked for.

If the symptoms persists despite the above treatment then an upper GI endoscopy should be planned in consultation with your doctor to see if any organic cause exists for the dyspepsia or to label it as functional dyspepsia and manage it accordingly.
