Treatment resistant dyspepsia requires further workup. Polycythemia , hypercalcemia , malabsorption and H. pylori manifestation/infection can cause such a presentation.
Empiric treatment with PPIs such as esomeprazole and prokinetic like itopride at full dose for 2 weeks should be attempted. H. pylori testing and eradication can be done to look for improvement. Offending drugs like NSAIDs, nicotine etc should also be looked for.
If the symptoms persists despite the above treatment then an upper GI endoscopy should be planned in consultation with your doctor to see if any organic cause exists for the dyspepsia or to label it as functional dyspepsia and manage it accordingly.
