Question: I sweat and wheeze and cough at night. I am already on HRT and do not have hot flashes during the day

I appreciate your concern

The primary indication of HRT is the vasomotor symptoms, after menopause. These include hot flashes and profuse sweating episodes. Since you are already on HRT, there are three things to keep in view. The therapy should work towards optimum lowest dose at which re evaluation should be done annually. At optimum dose , i.e at which symptoms disappear, one should continue HRT for at least 4 years. There could be dose adjustments required to achieve maximum effect in terms of the vasomotor symptoms.

The cough and wheeze are not related to the HRT or Menopause. A pulmonary and/or cardiac cause should be ruled out. Relevant workup should include a Chest x ray, EKG, PFTs, Echo, CBC.
