Question: I am 67 yrs old female I had lost 140 over a 2 yr period my husband of 45 yes died and they put me on ability and I gained 35 lb in one month and of course went quickly off the melds. Then in the last 2 years I have gain and additional 30 pounds because I couldn't effort my personal trainer who I had for a year. He was specially trained in rehab therapy. He left the area and I have tried 5 different trainers and they hurt me worse now I
have 2 rotator cuff tears from their improper training skills. I am having so much trouble getting that 65 lb off.
Most of the time food just doesn't taste good . I have trouble eating and I need to know what diet is best I am really confused I have tried glutton free look at the whole 30. If I just count my calories in will that work then I could eat a small about of what might taste good to me. I seem to do better eating 4-5 small snacks. I am not currently exercise and I know that does help. Can I lose just counting calories. I also have 4 artificial joints knees and hip and had rotator surgery on one shoulder and now they are talking artificial shoulder. I have depression all my life and anxiety and still grieving the lost of my husband. I just am struggling and of course my balance is not very good and I live in my home alone. I am having a problem with all the planned diet that you have to eat this and that at certain time. Now I was in a 11 car accident we were hit by a trash truck and have concussion and am being treated by 5 different doctors. I am frustrated and I hurt all over especially my head. Any ides?

Detailed Answer:
Hi! Welcome to health care magic!

It is sad to know about what you had gone through and I am grieved at your loss. You seem like a determined and brave person to deal with everything and multiple surgeries with courage. Still pursuing with tough exercises and rehabilitation is indeed commendable.

As for this moment, two issues have to be dealt concomitantly. First is the weight gain, despite calorie check and strenuous physical exercises. For this I would rather suggest that, since after rotator cuff injuries and multiple strains, refrain from extensive physical work out and concentrate on calorie intake. For that, simple method would be to take low carbohydrate and high protein meals with calorie restriction and make a diet plan with a nutritionist including dishes of your interest, to avoid monotony and boredom. Meanwhile to overcome probabilities of essential minerals and vitamins deficiencies you can take supplements and pharmacological replacements of B12, B6, B1, D3 and folic acid.

Secondly for the stressers, anxiety, depression and frustration you can join group therapies and take out time to meet psychologist for behavioural modifications and moods uplift. As you have had fought a lot and it is natural for every human to reach to a threshold where he or she needs a support to uplift his or her morale.

For the concussion, If any signs of drowsiness, vomitins, nausea, fits or alternating conciousness is experienced you would need to have an urgent neurosurgical consultaion through ER. At this moment, in absence of any such signs, you can take a routine followup with neurosurgeon and get fundoscopy and CT scan head done to look for any signs of concussion edema. If any such sign is observed it may need hospitalisation and relevant management with mannitol infusions.

Hoping you a speedy recovery. If you dont have any more questions, please close the discussion. You can also reach me directly through my profile in future, in case of any more questions, Regards.
