Suggest Treatment For External Hemorrhoids

Posted on Fri, 16 Dec 2016
Question: About four days ago, I developed what I believe is an external hemorrhoid. I am traveling out of state and did not have access to my doctor, so I self-diagnosed, and have been treating it with Preparation H and warm baths. It has not abated. A day after the hemorrhoid developed, the area in my lower left pelvis became distended and a large bulge is present. While the hemorrhoid is painful, the bulge is not, but it does not look right. I am concerned something may possibly be wrong with my colon or intestines. Your thoughts?

Hi.
Thanks for your query.
To recapitulate: Male/61 - self-diagnosed external hemorrhoid - painful - 4 days ago -
travelling out of state - Preparation H - warm baths - not abated - distension with a large bulge that is painless in the lower left pelvis - worried about wrong with colon or intestines - wants thoughts....

Please continue Sitz bath in a tub with warm water.
Take laxatives/stool softeners if the stool is hard. Try to push the external hemorrhoid if possible when lubricated with Preparation-H.
Do not do any activities that increase the straining like climbing up and down the stairs.

Please give some additional details for e to understand and assist you better:
Where exactly is the large bulge? (you have mentioned lower left pelvis)- Is it near the anal area or in the inguinal/groin area?
Does it increase on straining/coughing?

It is in the groin area immediately to the left and slightly above the penis.

Brief Answer:
Hernia/ lymph node as the commonest cause

Detailed Answer:
This may either be a lymph node mass or a hernia.
Hernia: there is an impulse/enlargement on coughing, it is reducible, painless and might have been present for a long time but noticed recently.
Lymph node mass: stationary, does not increase or reduce on doing anything.

Please check and let me know.

It is a hernia. I am sure based on your comments. I believe those are surgically repairable, so I will consult with my doctor upon returning home and ask for advice as to how to proceed. Thank you for the assistance.
