Brief Answer:
Get the fluid drained under ultrasound guidance

Detailed Answer:
Hi. Thanks for the query.
Congratulations first for being negative for lung cancer on CT.
On CT it must be pleural effusion I.e fluid between the two outer coverings of the lung. Once pus is aspirated then only confirmed diagnosis of empyema can be made.
At this juncture I can advise you to go for ultrasound guided fluid aspiration and if it is not pus then send it for following investigation.
Routine microscopy including protein, LDH, TLC, DLC, ADA, triglycerides, Malignant cytology.
If it comes out to be pus then has to be sent for culture sensitivity, CBNAAT for TB, AFB culture. And also this requires immediate drainage via intercostal chest tube drainage and empirical antibiotic therapy.
Sometimes severity of symptoms may be misleading. Cough and chest pain of any type may have underlying effusion.

Best wishes

I don't believe them.
That was his 20% theory.
Stabbing holes in people can cause secondary infections.

Especially in the hospital here, it's full of MRSA for one thing.

He does match TB and they didn't check for that.

Brief Answer:
Drainage of pus and antibiotic needed

Detailed Answer:
Well if he has stabbing injury then that may lead to secondary infection and at place where MRSA is high there are chances to develop empyema/pyothorax.
The treatment remains the same drainage of the pus as antibiotic won't help with pus in the body. And second thing antibiotic and culture sensitivity report. Empirically coverage should be given for MRSA like linezolid.
Best wishes
