Suggest Treatment For Fatty Liver And Cyst In Kidney

Question: Hi I had a general routing master checkup and in the process learnt

1. My liver size was enlarged - enlarged fatty liver of 15.7 CMs and Hepatic bilary system poorly visible
2. Evidence of cortical cyst in left kidney measuring 1.7*1.5 CMs

3. Total CHO/ HDL ratio was 3.8 ( <3.5 was diserable ) the rest of lipid profile was quite within range and so were AST and ALT levels( both around 20 . It is to be noted they were in 40s 2 months back and lost weight and was in diet to reduce it )

4. lastly there was 0.4 % presence of Immature Granulocyte in in WBC count. All other components were in range. Total WBC is 9500.

I took a abdomen scan 3 months back which neither showed a fatty liver or lidney cyst. SO I am worried that suddenly they show up.

Good morning,
I apologize for late reply.
Allow me to rule out first negative queries
1) Presence of immature granulocyte in blood is normal. Reference range for normal adult male is <0.50%. (WBC count within normal limit + normal immature granulocyte is absolutely normal)
2) CHO/HDL ratio of <3.5 (reduced risk) is possible only for athletic body with high metabolic rate. For normal adult male, routine lifestyle, regular diet, moderate exercise (cardiac exercise) - ratio of 3.8 is acceptable.
3) Mild fatty liver is reversible process. It is possible that if you do USG abdomen after 3-4 months then it might not be present. Fatty liver is deposition of fat in liver tissue due to higher load of fat content from food for metabolism in liver or defective metabolism of fat in liver.
4) Even LFT test would be normal in mild fatty change of liver except slight elevated triglyceride/total cholesterol level
5) To prevent further progression of fatty liver you need life-style and diet modification i.e. reduce fat content in diet (ghee, oil, butter), use of omega-3 fatty acid containing oil for food preparation, regular exercise, intermittent fasting, protein-rich diet.

To add further - My worry about IG in blood was mainly because it was never there in my blood before. I did a routine CBC 2 months back only to find that my WBC was 13000 range. Doctor who i consulted then ruled it out as minor infection , So I was very happy when I saw it back in the normal range. But IG panicked me. Do labs in general point IG only when they are present or only when they are abnormal. DO IG exist in perfectly healthy individuals?

Also regarding the fatty liver I heard there are grades in Fatty liver and based on the size I had just told will it be possible for your stage it. Again this scared me because in my previous ab scan the liver was normal. And in the last three months I have not exactly drunk or had any junk food.

Lastly I had submitted my full blood report and is there anything which I did not mention which you were able to identify that I should follow up.

Do not worry for IG

Detailed Answer:
Hello,
1) Mentioning of IG depends on which analyser was used by laboratory to examine blood sample and so it will not be mentioned in CBC done from every laboratory. No need to worry about IG as up to 0.5% IG are acceptable in healthy individuals.
2) Yes, fatty liver has grading regarding concentration of fat in it but it can be measured by only CT scan abdomen or MRI abdomen.
3) Sedentary lifestyle (in absence of alcohol, fat-containing diet) also responsible for fatty liver.
4) All your reports were within normal limit. But blood urea level and serum creatinine level were slightly towards higher side which indicate dehydration (less water content in body). So I suggest to drink fluids (water/juices) minimum 2lit/day.

Lastly - Is it advisable to take a CT scan based these results ? Also is cortical cyst only of benign in nature ?

No, based on your history and laboratory data, CT scan in not necessary.
But I suggest to do Lipid profile, LFT, USG abdomen after 3 months.
