Suggest Treatment For Fatty Liver

Question: Dear Doctor, I have fatty liver. The ultrasound says mild hepatomegaly with grade 2 to grade 3 fatty liver. I am obese and use to take alcohol daily. About 2 beers or 3 pegs of whiskey. Also used to take pan masala without tobacco. The insult has been there for about 10 years. My LFT were deranged earlier with SGOT as 65 and SGPT as 75. Now the SGOT is 32 and SGPT as 45 and GGT as 137. All other values are normal including Prothrombin time, lipids, hepatitis A,B,C.
My liver elastography shows fatty liver and no fibrosis.
The Fibroscan shows F3 fibrosis with stiffness values as 11.8 kpa.
The hepatologist says that F3 is reversible if the insult is taken away.
I am very confused about the difference of opinion between ultrasound ,elastography and that of fibroscan.
Kindly suggest and whether the situation is reversible.

I understand your concern.
well all the three test you mentioned have lot of limitations in accessing fibrosis.
Ultrasound is the poorest of all. It may miss significant fibrosis and at times even cirrhosis too.
Liver elastography if done by ultra sonography techniques is not so good too. If it is done by Magnetic resonance imaging it is better in finding fibrosis, cirrhosis but needs well trained experts to read it with significant inter observer difference.
Coming to fibroscan a value of more than 12 are usually associated with significant fibrosis which may actually be cirrhosis.
How ever all these have hot been very validated and absolute gold standard is liver biopsy.
Nevertheless all these values should not take you away from the fact that you need to seriously look into your liver problem and take following steps to curtail it and improve your liver functions.
1) Stop alcohol
2) reduce weight
3) avoid hepatotoxic drugs
4) exercise regularly
5) reduce dietary fat intake too
Some times the reports of all these three may vary from the actual amount of liver damage that you have hence it is advisable to take all the possible precautions to limit further liver damage.
In addition liver fibrosis is reversible till certain extent only and that too if the insult to liver is completely removed .

Thanks for your reply.

If I take my condition very seriously and work on the goal, assuming its F3 fibrosis, do you think its reversible?

And with fibroscan value of 11.8 do you think I am cirrhotic?

And how much maximum weight loss would you advise in a month?

See definitely some degree of fibrosis is reversible but not all.
Also you need to arrest it further damage.
Value of 11.8 may not suggest cirrhosis but significant fibrosis is there with a very high risk of progression to cirrhosis.
Weight loss should be slow and steady around 2 kg a month. To reduced your body mass index to below 24 kg/mt2.
Need to monitor your liver functions too.

Dear Sir,

Final question.

With my care taking and weight reduction is there a good probability of fibroscan values going down?

I am also very concerned as I am a pilot by profession and whether fibrosis can put me medically down?

I have been advised Ursocol 300 mg twice daily and Vitamin E 400 mg once a day.

Lastly what are first warning signs that my liver is worsening?

Yes there is a good probability of your values going down with weight reduction, diet control and most importantly stopping alcohol.
I am not sure about flying guidelines in terms liver fibrosis. But it will definitely affect it if it is an advanced problem and if picked on ultrasound.

Need to increase the dose of ursocol after consulting your gastroenterologist.

Some warning signs need to be looked into urgently will be swelling of feet, distention of tummy, jaundice.
