Question: Hello,
I have difficulty going out i want to go but i am worried that i will burst into tears. This has happened so many times. Then i pluck up the courage to go out again, but this last episode has left me never to go out again. i love going out, but i just feel angry with myself for breaking out in tears, because i feel happy underneath. could this be PBA. In the past i thought it must be depression, but depression drugs have not helped me. Appreciate your advice.

In depression patient may cry and remain sad most of the time unlike in your case. Having difficulty in going out and feeling comfortable inside house occur in social anxiety and agoraphobia (type of anxiety disorder).

Anxiety occurs occurs due to neurotransmitters imbalance in brain most commonly involving serotonin neurotransmitter.

There is need for treatment, if symptoms cause significant personal distress or affect personal/professional life (like in your case). Both drug and non drug treatment like cognitive behaviour therapy (form of psychotherapy) are used for treatment. Choice of treatment depends on severity of symptoms, personal preference and availability of treatment.

In drug treatment two types of medicine used. For immediate relief of anxiety benzodiazepines group of medicines are used and for long term relief most commonly SSRI (selective serotonin reuptake inhibitors like escitalopram/paroxetine) are used. SSRI takes around 3-4 weeks to show its effect. Non drug treatment like cognitive behaviour therapy (form of psychotherapy) also takes weeks to show its effect.
