Based on your query, my opinion is as follows:
1. Temperature of 103 degrees is high and needs to be controlled with paracetamol.
2. It appears to be viral fever.
3. Due to high temperature, he has become dehydrated and this will be the cause of fainting and fatigue. Also, nutrition requires improvement.
4. Unless other symptoms, if any are present, difficult to identify, it as an insect bite cause. Numerous protozoal, bacterial and even viral infections can occur after insects bite. Recent bite, if any specifically known, can be thought of. Bite area, if any swelling or redness, needs to be looked into. However, at present it appears to be viral only. Paracetamol 3-4 times daily should suffice in controlling the symptoms.
5. No clinical or laboratory investigations may be necessary for now. If any additional symptoms or if fever persists after 3 days or if fever does not come under control with paracetamol, you will need to go to ER.
6. At present, more fluid intake, as dehydration is high (At least 6-8 liters or even more of water) along with protein-rich diet is necessary. Also rest and paracetamol for fever should help.
