Brief Answer:
most cases are due to infection but if pain very bad should be reassessed

Detailed Answer:
HI, thanks for using healthcare magic

Viral or bacterial infection are the most common causes of acute diarrhea though there are other causes as well.

The presence of fever, however, makes infection the most likely cause. Most persons can be treated at home but if the pain is very severe then you should either go back to the ED or visit your personal doctor for an assessment to rule out any other causes, in some cases , antibiotics may be needed.

Most infections of the gastrointestinal tract can be treated by: (1) holding off all solid food till there is a 4 hour window with no symptoms

When this 4 hour window occurs then start with very small amounts of dry food such as toast, biscuits, plain rice etc

(2) probiotics help restore the normal bacteria in the colon. These can be used on their own or in yogurts

(3)fluids are very important, you are losing fluids via the stool , previous vomiting. This needs to be replaced. Diluted sports drinks or oral hydration fluids are best

(4)medication such as pepto bismol may help with the diarrhea and nausea (if this remains)

Continue the pain medication

Infections can last up to a week in some persons but the symptoms should be progressively decreasing.

I hope this helps, feel free to ask any other questions
