Detailed Answer:
Hi,

Thank you for your query. I can understand your concerns.
Fever with shivering may be due to urinary tract infection (common in female) or some deep seated abscess like liver abscess. You need to undergo routine urinalysis and urine culture & sensitivity tests apart from ultra sonography of abdomen & pelvis. Your TLC shows leucocytosis (TLC-11,400/c. mm with neutrophils 94%) indicating bacterial infection. A normal x-ray chest points towards non-respiratory cause of infection.
Your ESR is high (105 mm) but it may be age-related (ESR increases with age and over the age of 60 years ESR of 20 mm or more may be present without any obvious cause) but collagen vascular disease and chronic infection like tuberculosis needs exclusion.If possible upload your complete hemogram.
Do you have any past or family history of tuberculosis?
Azithromycin is a macrolide anibiotic and it can itself cause stomach upset(flatulence,dyspepsia etc).
The further course of treatment will mainly be dictated by detailed history and clinical findings.
Laboratory investigations will remain supportive.
