I think you have large leg swelling and it has cosmetic problem. The management is about grading of the lymphodema. It is called elephantiasis which is a complication of lymphatic filariasis. This stage is irreversible and cannot be treated back to normal size and shape.
Since it is the lymphodema and the sub cutaneous fibrosis, it may require surgical management. A cosmetic surgeon can help you in chennai in such a case.
Do not lose hope you should get him treated with acute attacks with antibiotics.
