Brief Answer:
Azithromycin is right choice.

Detailed Answer:
Hello,
Welcome to HCM.
I am Dr Rakesh Sharma answering your query.
I can understand your concern.
Back ache and chest pain are common symptom of Pneumonia.
But one should also keep in mind a muscular origin.
If she has fever as well, you can add ibuprofen, if she has no liver or kidney issue.
Give hot compress at back or a heating pad will be of great help.
If I was her treating doctor, I would have started with Azithromycin 500 once a day.
Use diapers for perineal hygiene, as diabetics develop bed sores earlier than nondiabetics.
Hope I have answered your query.
If any thing else you are welcome.
Good luck.
