Question : Hello, I'm a 50 year old female and I've had a hemmorhoid for years with mild symptoms (occasional itching mostly); and the one other result is the smell of feces (even to the light touch) at my anus entry, even when I am just out of the shower or bath. I feel like it is getting a little more progressive and I would like to consider treatment to eliminate this embarrasing smelly inconvenience. I appreciate any suggestions you might provide! Thank you.

Detailed Answer:
Hi,

I read your medical history carefully and understood your concerns.

Hemorrhoids are itchy only in following cases:
- during healing process (as the mucosa is being regenerating)
- of infection especially fungal infection

With regards to smelling, hemorrhoids themselves do not smell. What smells is the fecal mass that is entrapped around the hemorrhoids and are difficult to clean. That's why keeping good hygienic measures will help reduce such smelling:
- wash with warm water and soap
- avoid harsh tissue;
- instead, I encourage to use soft tissue or wet cleaning wipes

In order to treat hemorrhoids permanently, I advise to consult further with proctologist as a thorough rectal examination is needed and proctoscopy to evaluate the changes and the size of hemorrhoids to decide if surgery is the best option in your case.

Culture of stool and examination of feces for ova/parasites are needed to rule out bowel infection (bacterial/fungal) and/or intestinal parasites.

Avoid constipation, that will aggravate hemorrhoids.
Go to bathroom each time you feel the need; do not delay. This will prevent constipation and further aggravation of hemorrhoids.

You can discuss my suggestions with your proctologist for further evaluation and treatment.
