Brief Answer:
Yes you need to go to an E.R soon

Detailed Answer:
Hello and Welcome

I appreciate your concern

There is marked swelling at the junction of the metarsals and the phalanges. Abrasions are also present. X rays of the foot would be required to look for a fracture. If the fractured bones aren't badly displaced then immobilization through casting will suffice. Open reduction and internal fixation would be required if the are severely angulated or displaced. A tetanus toxoid booster shot should be given if vaccination is incomplete or you are not sure about vaccination status. Diclofenac sodium I.M can be given for the associated pain and swelling. Please go to an E.R near you for further manage

Wishing you best of health

Thanks
