Suggest Treatment For Frequent Bowel Movement And Anal Pain

Question: I am a 68 year old female that over the passed year have experienced more frequent bowel movements sometimes 8 to 10 a day sometimes with anal pain and sometimes with extreme urgency. My stool is normal and I have no other symptoms. It has become so annoying and I am always looking for where the bathrooms are located with I go out anywhere.

I forgot to add that I have a sedentary job where I sit a desk all day.

You are reporting to have experienced more frequent bowel movements over the passed year. If I was your caring doctor would advise to run further diagnostic tests to determine the right cause:
- stool culture
- examination of stool for ova/parasites
- abdominal ultrasound
- malabsorption testing

Depending on the results, specific treatment will be prescribed. If everything would come ok,then, IBS is to confirm and start proper treatment.

For the moment, I advise to:
- avoid constipation
- try not to stay sit for a long period of time
- take PPIs and antacids for stomach pain. Buscopan or papaverin to ease the pain
- be careful with foods/drinks that would aggravate you symptoms
- increase water intake

My primary care doctor told me last year I did have a touch of IBS to increase fiber and water intake. Is there anything I can do to help cut down on the constant need to have a bowel movement?

If you have been diagnosed with IBS, then, all your symptoms are related to IBS. You need a good treatment strategy to handle IBS symptoms you are currently having.

The very first thing to handle your symptoms includes diet changes:
- avoid those foods that would aggravate the symptoms: cabbage, broccoli, beans, cauliflower, raw foods, etc.
- avoid foods that contain gluten (wheat, barley, rye) and those high in fructose, fructans, lactose, etc.

You are already taking fiber and water, which are also helpful in handling your symptoms.

If that is not enough, then, I advise to discuss with your GI specialist about adding digestive enzymes and simethicone for the first weeks to improve digestion and your symptoms consequently.

Controlling stress/anxiety is also very important to control your bowel movement. Stress relieving techniques like deep breathing and walking in fresh air can also be helpful.

Probiotics can be useful in controlling IBS symptoms. However, sometimes it could be associated with bacterial overgrowth. That's why when prescribing probiotics, I advise my patients to observe the decurse of symptoms : if improving or not.

Can you give me some suggestions on foods vegetables and fruits that I can eat as opposed to what to avoid? I have started A food journal to see what happens after I eat but I could use some guidance on "good" fiords and drink. Thank you

Sorry for the late reply due to my emergencies in the clinic.

Please find the following as sample diet to control your IBS symptoms:

Breakfast: Scrambled eggs with herbs accompanied by a smoothie (combination of banana, spinach, water and almond butter)

Lunch: Split pea soup with whole grain roll.

Dinner: grilled chili and cilantro salmon with ginger rice

This is only a sample of what you can eat during a day. However, you should pay attention as your bowel might get a bad "reaction" to a specific food and can trigger IBS flare up.

Do not forget to drink plenty of water during the day.
