Suggest Treatment For Fungal Infection Around Groin Area And Back

Question: Hi, i had fungal infection around groin area and back. i took teribinafin for a week as suggested in my previous query. it is fine now, but getting itching at times mostly at night and few red marks. I am applying candid B over couple of times daily. Is it fine to continue with this treatment or reinforce with any other medication. will the infection recede in coming days.

Welcome again and thanks for posting your details along with pictures.

I can get that your condition of fungal infection is quite better now. Before I guide you regarding further management let me guide you certain important points which are useful in further management.

To guide you simply regarding this condition: Fungal infections are common type of infections which can affect anybody at any site. But more commonly it affects the areas which are more prone for sweating or moist areas (as in your case).

More specifically these are related to the hygiene and environmental factors along with clothes which we wear. As the thigh area is continuously closed so it predisposed to moistness at this area.

Due to hot and humid climate or due to tight and synthetic clothing, it leads to growth of fungus at these parts of the body.

I suggest certain more suggestions as below:

- Keep yourself away from hot and humid climate.

- Take two times bath.

- Wear cotton clothes and cotton undergarments to be changed two times a day. Do not wear any synthetic or tight garments.

- Do cold compression or warm water compresses with towel soaked in water according to the weather condition.

- Do not sit continuously for long hours.

-Don't try to scratch or rub the area as can lead to further aggravation of the problem.

- You can use any good antiperspirant powders daily to avoid future infection.

- Reduce your weight if it is on higher side.


You have been given good treatment but there are certain import things to be guided:

- Terbinafine once started must be taken for atleast 3 -6 weeks for complete eradication of fungus otherwise it recurs back.

- Candid-B cream contains steroid and is to be applied only on inflammatory areas or red areas and not beyond 5 days then on alternate days for next 6 days and then twice a week before stopping completely.

It should be replaced by plane clotrimazole cream (Candid cream and not Candid B).

The creams provided are to be applied for two times and not multiple times over affected area only. It should be applied very thinly so that it should not be visible to the naked eye. I guide my patients to apply a pea size amount of cream to be sufficient for palm size area. Do not rub but gently massage the area.

For itching sensation: I prefer to give Des-loratadine tablet for two times. In case of itching at night time then can be taken after dinner time.

Thanks a lot for your suggestion and guidance. i will continue terbinafine for another 2 weeks. i will follow other guidelines as well. I will switch to candid from today since i had used candid B for 9 days now. for the antiperspirant powder, will 'shower to shower ' or ' nice' be good enough or is there any specific powders. also how long do i continue the medicine for itching? one more important thing i would like to know is about the contagious nature of this infection. will it spread quickly to other parts of the body and to other person as well. i have a 3 yr old son at home and would like to hear some guidance on this as well. just wondering how this infection suddenly appeared: i went for midnight marathon and came home, went straight to bed? or i wore a very old under garment which i wore when i had some mild infection around 3 yrs ago which was treated with 'itch guard' cream itself. does the fungus remain inert for so long?? sorry for a long chain questions.. thanks in advance for your answers

You are most welcome to ask as many queries as you want and can take suggestions accordingly.

For your further queries;

- I generally use Candid TV powder or Canesten dusting powder. It is to be sued at the areas which have healed properly and helps in maintaining less moisture at the affected area.

- The medicines for itching can be continued as long as you perceive the same. As soon as you feel the itching has reduced then can take one tablet or on alternate day basis also. So once the itching has subsided then the tablet can be taken "As and when required basis."

- Yes, fungal infections are quite contagious in nature and can spread from one part to other part as well as form person to other. Therefore for the prevention it is better to keep your belonging separate. I additionally suggest you to iron your all the clothes and undergarments from inside. It burns out the fungus and reduces the chances of spread. Expose your bedsheets and towels in direct sunlight daily.

-- You are correct in observation that it got started by your improper hygiene after the marathon and by wearing old undergarments which are both the source of fungal infection.

- Fungus is present in the environment everywhere. It gets access only during the time when the moisture is on higher side which might had happened after the racing. So maintain good hygiene. Do not use itch guard cream as can burn the skin.
