Suggest Treatment For Fungal Infection At Interpol And Upper Pole Of Kidneys

Question: My wife is SLE patient from 9 years. She got fungal infection at inter pole and upper pole of both kidneys and culture of pus aspergillus fumigatus. Caspofungin given 14 doses, liposomal amphotericin 46 doses I.e 4 mg /kg/day and at present on voriconazole 400mg/day from 8 weeks. The size waswas almost same at start of voriconazole to this time. What I can do and which antifungal is suitable for her complete cure of fungal infection

I think you have taken all the highest anti fungals available. Caspofungin is one of the newer formulation while Amphotericin is one of the best drugs available. I would like to know if you repeated the cultures again to see if it is negative?The fungal infection would have gone off by now with these drugs ,or else it would have subsided to a level of causing no symptoms.

1. Could you please describe or attach a report of the ultrasound which shows the swelling at the poles of the kidneys? i have my doubts on it being fungal in the first place

2. What did the cultures show sensitivity too??

3. IS it on both the kidneys or just one? Her serum creatinine is normal??

4. What are the medications for SLE she is taking??

DO let me know these queries. Any scans or ultrasound to show the persisting infection would be helpful
