Suggest Treatment For Gas Formation And Stomach Upset

Question:
Two issues -
1. Related to digestive system
Symptoms are Gas formation ,
Stomach upset , gas pass, foul smell, belching , low grade fever,(97.4) frequent loose/ semi loose motions
2. Curvical spondylitis -
Symptoms
Right side movement leads to chakkar( dizzy) feels as if things are moving in circular motion , likewise om getting up from the bed& bending down

Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.

Looking at the clinical history and query, my opinion is as follows:

1. You are on quite a lot of medications. I am not sure, you are taking all of them at present. Are you taking Dulcolax? and semi loose motions should be normal. Do provide me the current list.


2. Cervical spondylitis can cause pain in the neck with radiation to limbs. You need to wear cervical collar for it and also should try physiotherapy.

Movement of head and dizziness points towards a possible vertigo. However, especially getting up from bed or bending down is postural hypotension.

It is due to old age and reduced baroreceptor adjustment.
You need to move around slowly, wake up slowly, give time body to adjust. If necessary you can try stockings to avoid pooling of vein in lower limbs.

You can get a confirmation by getting your Blood pressure evaluated during getting up from bed. Slow movement is necessary.
Avoid falls at this age. Also check your hemoglobin levels, so that improved oxygenation can lead to less dizziness.

All the medication listed is the present medication.

Kindly suggest if any diagnostics are reqd?

Stomach issues( gas formation n belching) have been going on for past 10 yrs however the loose motion and frequency of motion has started only in past 45 days.

The doctor had initially mentioned the intestine valve had gone weak/ dysfunctional and hence belching etc

Dehydration leading to dizziness.

Hi,
Thanks for updating me. I went through the list.

It is really disturbing to have loose motion and increased frequency of motion at this age. The possible associated dehydration is leading to your dizziness symptoms.

Keep yourself well hydrated. Avoid drinking too much water at night, as it can disturb sleep.

Gas formation, foul smelling is usually due to inadequate digestion of food.

You need to do the following:
1. Slow chewing of food and thoroughly.
2. Multiple small meals. As you are having large meal, it is frequently pressing on the upper end of esophagus and mostly not getting digested, you are having gas formation.
3. Avoid talking during food intake, as to avoid taking too much air inside into gut.
4. You need to take yoghurt or probiotics to improve gut bacteria. Taking it on a regular basis will improve the gut flora and reduce gas formation.
5. Try to take soluble fibers in your diet. They could be oatmeal, oat cereal, lentils, apples, oranges, pears, oat XXXXXXX strawberries, nuts, flaxseeds, beans, dried peas, blueberries, psyllium, cucumbers, celery, and carrots.
6. Insoluble fibers have a laxative effect and also would not get digested increasing the incidence of gas formation.

Investigations, at present importantly are
1. Stool examination for blood and fat testing.
2. Routine hemogram as you could be anemic or there could be infection.
3. Colonoscopy to evaluate for lesions in the large intestine.

Belching is the other route for air to expose, if not from below. If its in stomach, it comes out as belching due to increased ingestion of air during food intake or gastritis. Beyond that, it usually forms gas.

Drink lots of fluids, it will reduce your dizziness. Move around slowly, as any falls will be serious at this age.

Do watch for blood in stools. Follow the few things i mentioned earlier. Be well hydrated. Get the investigations done.
