Question: HI I had gastritis about a month ago the symptoms went away then I took some tynol extra strength ok Thursday and I been having sharp pains in my upper abdomen and loss appetite I've been using the nexium but I still feel pain when I wake up and wheb I lay down ...

I appreciate your concern

Based on your description, it appears to be another episode of gastritis or GERD. If the epigastric pain is persistent despite taking the nexium then the dose should be adjusted. 40 mg of esomeprazole can be taken twice a day for gastritis or GERD despite using nexium. Pro kinetic medication like metoclopramide or domeperidone and sucralfate can also be added to this regimen for added control, initially. Serum Amylase and lipase levels can be done to rule out possibility of pancreatitis. If there are recurrent episodes of such abdominal pain and discomfort then an upper GI endoscopy would be the way to go in consultation with a gastroenterologist.

Hello again.

No, its not normal to have these symptoms despite being on esomeprazole. As I stated above the PPI dose needs to be given in maximum recommended amount and if the symptoms persist then an endoscopy should be done to rule out any sinister cause.
