Brief Answer:
Persistent pubertal gynecomastia could be the cause

Detailed Answer:
Hello. Thank you for writing to us

I am dr.kakkar (dermatologist). I have gone through your query and I have noted your concern.

I would keep a possibility of Persistent pubertal gynecomastia.
It is thought to be caused by an altered ratio of estrogens to androgens; either an increase in estrogen production, a decrease in androgen production, or a combination of these factors. This can occur because of expected hormonal changes during puberty. This condition may persist into adulthood.
Obesity can also be associated with increased peripheral conversion of androgens to estrogens and is associated with a higher prevalence of gynecomastia.
